Job Title: Licensed Practical Nurse (LPN) - PRN

At Allegiant Home Care, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a PRN (as needed) nurse. As an LPN, you will provide high-quality nursing care to our patients in the comfort of their own homes.

Responsibilities:
  • Provide nursing care under the supervision of a Registered Nurse and in accordance with the patient's plan of care, law and regulation, standards and scope of nursing practice, and Allegiant Home Care policies and procedures.
  • Monitor and report changes in patient condition to their supervisor.
  • Implement plan of care as determined by the DON, RN, and the Case Care Team and under the scope of authority to practice.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Provide patient and family caregiver education and information pertinent to the diagnosis and plan of care.
  • Assist patient in learning appropriate self-care activities.
  • Use equipment and supplies effectively and efficiently.
  • Under the direction of the DON, is responsible for maintaining compliance with all state and federal program requirements, obtaining and maintain certification under CHAP, CMS, and any other state or federal law or rules.
  • May participate in Case Conference, actively communicating relevant issues to the team, ensuring all team members are informed and up to date, may participate in the development of the plan of care, and monitor and articulate patients' progress or decline.
  • May participate in all quality assurance committees as part of the overall quality improvement program.
  • Maintains professional affiliations and any required certifications.
  • Provide administrative on-call coverage on a rotation basis.
  • Ensure that patients' database is maintained and updated.
  • Perform related nursing duties as required or requested.
  • Assists in the nursing treatment, monitoring, and testing of patients.
  • Observes, records and reports patient's condition and reaction to drugs and treatments to RN Supervisor.
  • Attends staff meetings and in-service training as required.
  • Follow all Policies, Procedures, and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Protect privacy and maintains confidentiality of all company procedures, results and information about employees, patients, and families.
  • Practice Standard precautions and follows Allegiant Home Care Infection Control procedures.
  • Participate in organizational committees as identified that enhance the working relationships between departments within Allegiant Home Care.
  • At all times, represent Allegiant Home Care professionally through care and/or services provided to all clients and patients.
  • Comply with Allegiant Home Care policies, procedures, and standard practices.
  • Maintain the confidentiality of patients and families within Allegiant Home Care as per government mandates.
  • Promote Allegiant Home Care through participation in community and professional organizations.
  • Improve own professional knowledge and skill level.
  • Additional duties, as assigned.
Qualifications and Requirements:
  • Graduate from an LPN education program.
  • Florida state licensure to practice as an LPN.
  • Valid Florida driver's license and active auto insurance.
  • At least 1-year of experience in a home health setting, preferred.
  • Current CPR certification.
  • Skilled at:
    • Knowledge of principles, practices, standards, and techniques of nursing.
    • Demonstrated effective decision-making skills.
    • Ability to perform diversified nursing duties with time limitations with high degree of accuracy.
    • Ability to foster collaborative working relationships.
    • Working knowledge of physical, mental, spiritual, transportation, and social needs of the patients and their families.
    • Ability to apply creative problem-solving skills.
    • Ability to plan, prioritize, and follow-up on patient plans of care effectively and efficiently.
    • Effective written and oral communication skills.
    • Knowledge of safety requirements in nursing.
    • Requires individual who can coordinate multiple tasks simultaneously.
