Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health services to patients in Jacksonville, FL. As a LPN, you will work under the supervision of a Registered Nurse and in accordance with the patient's plan of care, law and regulation, standards and scope of nursing practice, and Allegiant Home Care policies and procedures.

Key Responsibilities
  • Implement plan of care as determined by the DON, RN, and the Case Care Team and under the scope of authority to practice.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Provides patient and family caregiver education and information pertinent to the diagnosis and plan of care.
  • Assist patient in learning appropriate self-care activities.
  • Use equipment and supplies effectively and efficiently.
  • Under the direction of the DON, is responsible for maintaining compliance with all state and federal program requirements, obtaining and maintain certification under CHAP, CMS, and any other state or federal law or rules.
Requirements
  • Graduate from an LPN education program.
  • Florida state licensure to practice as an LPN.
  • Valid Florida driver's license and active auto insurance.
  • At least 1-year of experience in a home health setting, preferred.
  • Current CPR certification.

  • Knowledge of principles, practices, standards, and techniques of nursing.
  • Demonstrated effective decision-making skills.
  • Ability to perform diversified nursing duties with time limitations with high degree of accuracy.
  • Ability to foster collaborative working relationships.
  • Working knowledge of physical, mental, spiritual, transportation, and social needs of the patients and their families.
