Job Title: Care Management Extender

Vaya Health is seeking a highly motivated and compassionate Care Management Extender to join our team. As a Care Management Extender, you will play a vital role in supporting care managers in delivering tailored care management services to our members.

Job Summary:

The Care Management Extender will work closely with care managers to provide outreach, support, and education to members, as well as coordinate services and appointments. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:
  • Perform general outreach, engagement, and follow-up with members
  • Coordinate services and appointments, including transportation arrangements
  • Engage in health promotion activities and knowledge sharing, such as providing education on chronic conditions and self-management skills
  • Share information with care managers and other team members on member circumstances
  • Provide and track referrals, and assist with obtaining and maintaining community-based resources and social support services
  • Participate in case conferences and support care managers in assessing and addressing unmet health-related resource needs
Requirements:
  • High school diploma or GED required
  • Must be at least 18 years of age and trained in Tailored Care Management
  • Must meet one of the following requirements: lived experience with an I/DD or TBI, certified peer support specialist, parent or guardian of an individual with an I/DD or TBI, or two years of paid experience performing similar functions
Preferred Qualifications:
  • Certified PSS (Peer Support Specialist) and/or WRAP (Wellness Recovery Action Plan) Facilitator
Working Conditions:

This is a remote position, and the successful candidate must reside in North Carolina or within 40 miles of the NC border.

Salary and Benefits:

Salary will be commensurate with qualifications and experience. This position is non-exempt and eligible for overtime compensation.
