Responsibilities:


The registered nurse is responsible for planning, organizing, and overseeing hospice care, with a strong focus on community health education and experience.

This professional role leverages community resources to design and implement services tailored to the needs of individuals and families in their homes and communities.

Essential Job Functions/Responsibilities

Conducts comprehensive initial and ongoing assessments of patients and families to identify hospice needs. Performs thorough physical assessments and gathers detailed medical histories.

Delivers professional nursing care by applying all components of the nursing process.

Evaluates and monitors the patient's condition by: (A) Developing and initiating a care plan (B) Regularly reassessing patient and family/caregiver needs. (C) Modifying the care plan as necessary.

Utilizes health assessment data to formulate nursing diagnoses.

Creates a care plan that sets goals based on nursing diagnoses and incorporates palliative nursing actions, involving the patient and family in the planning process.

Implements appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as directed by the physician's care plan.

Provides counseling to patients and families to address nursing and related needs.

Offers health care instructions to patients as deemed appropriate based on assessments and care plans.


Assists patients with daily living activities and supports their journey towards self-sufficiency and optimal comfort care.


Acts as Case Manager when designated by the Clinical Supervisor/Nursing Supervisor, taking responsibility for coordinating patient care for an assigned caseload.

Communication

Maintains accurate and relevant clinical documentation regarding patient conditions and care provided. Records changes in pain/symptom management as appropriate.


Engages with physicians regarding patient needs and reports any changes in conditions; obtains and processes physician orders as necessary.

Collaborates with community health professionals to coordinate care plans.

Educates patients and families/caregivers on self-care techniques as appropriate.

Provides medication, dietary, and other instructions as prescribed by the physician and identifies opportunities for health counseling with patients and families/caregivers.

Works collaboratively with the interdisciplinary team.

Ensures a safe environment for patients.

Assists patients, families/caregivers, and team members in ensuring continuity of care.


Collaborates with families/caregivers and hospice Interdisciplinary Group Members to address the emotional needs of patients and families/caregivers.

Participates in interdisciplinary group meetings.

Additional Duties

Engages in on-call responsibilities as outlined in the on-call policy.

Ensures that necessary equipment and resources are available.

Supervises support staff and delegates responsibilities as needed.


Takes charge of personal growth and development, maintaining and enhancing professional knowledge and skills through continuing education and in-service training.

Meets the obligations of accepted case assignments.

Actively participates in quality assessment and performance improvement initiatives.

Qualifications:
Registered nurse with valid licensure to practice professional nursing.

Graduate of a nursing program accredited by the National League for Nursing.

Current CPR certification required, adhering to American Heart Association BLS Guidelines.


Must possess a valid driver's license and have access to a reliable, insured vehicle.


Registered nurses should have a minimum of (a) one (1) year of professional nursing experience within the last three (3) years OR (b) hold a baccalaureate degree in nursing from an accredited program and possess a current RN license.

Preferred: Minimum of two years' experience, with at least one year in public health, home care, or hospice nursing.

Self-motivated and capable of working independently.

Management experience is not required, but supervision of hospice aides is expected.

Demonstrates strong observational, problem-solving, verbal, and written communication skills; nursing competencies as per checklist.

Ability to organize and prioritize workload independently.

Physical requirements include prolonged walking or standing, lifting, positioning, and transferring patients, as well as lifting supplies and equipment. Requires considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing are necessary to perform nursing skills.
