We are in search of a dynamic, empathetic, and detail-oriented Registered Nurse (RN) to take on the role of Hospice Case Manager for our clientele.
As Hospice RN Case Managers, you will be responsible for planning, organizing, and directing hospice care through the nursing process, which includes assessment, planning, interventions, implementation, and evaluation. Key Responsibilities:
Empower Through Engagement:
Respond to clinical referral information with expertise and compassion, addressing inquiries and requests with a service-oriented approach that reflects our dedication to exceptional care.
Collaborative Care Champion:
Work closely with our Clinical Director to evaluate the eligibility and appropriateness of clients for our specialized hospice services. Your expertise will be instrumental in ensuring timely and appropriate care.
Nurturing Direct Care:
As an RN Case Manager, you will not only coordinate care but also serve as a source of comfort and support for patients, prioritizing their comfort and quality of life.
Nursing with Heart:

Qualifications:
• Hold a current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; One (1) year of experience as a professional nurse within the last three (3) years.
• Must possess proficiency in computer skills, including typing and familiarity with various programs, particularly EMR.
• Maintain current CPR certification.
• Compensation is set at $58.00 per hour (Average Pay Range), with pay determined based on experience.
