Facility Information:
La Paz, a part of the Telecare Family, is a 148-bed Gero-Psychiatric center that caters to adults aged 55 and older.
Available Shifts:
AM: 7:00 am - 3:30 pm
PM: 3:00 pm - 11:30 pm
NOC: 11:00 pm - 7:30 am
Full Time; shifts and days may vary based on operational needs.
Compensation:
The anticipated starting wage is $24.31, with geographic differentials applied to pay ranges based on location. Starting pay will be determined by relevant experience.
Position Overview:
The Certified Nursing Assistant plays a crucial role in Skilled Nursing Facilities, addressing both the physical and psychological needs of the individuals served, in alignment with recovery-centered care principles.
Qualifications:
Required:
- High School Diploma or GED
- Current Nursing Assistant Certification
- Proficient in English communication, both verbal and written
- Minimum age of 18 years
- CPR, Crisis Prevention Institute (CPI), and First Aid certification required upon employment or within 60 days
- Successful completion of a post-offer physical examination, tuberculosis testing, background checks, and verification of credentials is mandatory.
Preferred:
- Experience with the target population or in a similar program setting
- Bilingual in threshold languages
Key Responsibilities:
- Exemplify the mission, purpose, values, and beliefs of Telecare in all interactions with stakeholders.
- Assist with the welcome, admission, and discharge processes.
- Support and guide individuals in daily living activities.
- Ensure the safety of individuals through vigilant monitoring and documentation.
- Assist individuals during mealtimes.
- Participate in community meetings and group activities.
- Facilitate rehabilitation therapy groups and activities as needed.
- Implement care plans effectively.
- Contribute to a recovery-oriented environment through positive interactions.
- Collect specimens as required.
- Measure vital signs and weights as assigned.
- Adhere to standard precautions consistently.
- Engage actively in multidisciplinary team meetings and treatment planning.
- Maintain confidentiality and integrity regarding client information.
- Demonstrate knowledge of CPI principles and techniques.
- Report observed behaviors and conditions to the charge nurse.
- Document and communicate changes in client behavior effectively.
- Assist in the assessment process during admissions.
- Provide support in crisis situations as necessary.
Skills Required:
- Basic computer skills for electronic health records.
- Strong verbal and written communication skills in English.
- Ability to interact effectively with diverse personalities and situations.
Physical Requirements:
The physical demands of this role include the ability to sit, bend, squat, kneel, crawl, climb, and lift items weighing up to 25 pounds. Frequent walking, standing, twisting, reaching, and grasping are also required. The position may involve exposure to hazardous materials and requires manual dexterity.
