Job Title: LVN - Congregate Living

We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our team at Excell Home Care Inc. in providing exceptional care to our residents in a congregate living facility.

As an LVN, you will be responsible for ensuring the continuity of care between shifts by providing direct care and supervising the care given by Certified Nursing Assistants (CNAs) and support staff members.

Key Responsibilities:
  • Provide patient care in accordance with facility policies and procedures
  • Monitor and record vital signs, including temperature, pulse, and blood pressure
  • Assist with dressing wounds, inserting catheters, and collecting lab samples
  • Communicate effectively with residents, families, and healthcare team members
  • Accurately document resident progress notes and report any changes in condition to the Nurse Supervisor
  • Assist CNAs with daily care activities and ensure equipment is in good working order
  • Monitor residents at risk for falls and take necessary precautions
  • Follow Infection Control policies and report any occupational exposures to blood or bodily fluids
  • Attend annual facility in-service training programs and complete additional assignments as requested by management
Requirements:
  • Current LVN licensure in the state of employment
  • Ability to work flexible hours, including mornings, midday, and night shifts
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Basic computer skills and familiarity with electronic medical records
