Key Responsibilities- Develop individualized care plans that reflect clients' preferences and requirements.

- Evaluate client progress and modify care plans as needed.
- Facilitate communication among clients, families, and healthcare professionals to keep all parties informed about the client's status.
- Maintain precise and current documentation regarding clients and caregivers.
- Participate in an on-call rotation to provide after-hours support when necessary.
- Adhere to all company policies and safety protocols.
- Current LPN license.
- A minimum of 2 years' experience in home healthcare or a related field.
- Excellent communication and interpersonal abilities.
- Proficient in assessing and analyzing situations promptly.
- Familiarity with Medicaid and Medicare guidelines.
- Capability to work independently as well as collaboratively.
- Willingness to engage in on-call duties.
