Key Responsibilities
  • Develop individualized care plans that reflect clients' preferences and requirements.

  • Facilitate communication among clients, families, and healthcare professionals to keep all parties informed about the client's status.
  • Maintain precise and current documentation regarding clients and caregivers.
  • Participate in an on-call rotation to provide after-hours support when necessary.
  • Adhere to all company policies and safety protocols.
Qualifications
  • Current LPN license.
  • A minimum of 2 years' experience in home healthcare or a related field.
  • Excellent communication and interpersonal abilities.
  • Proficient in assessing and analyzing situations promptly.
  • Familiarity with Medicaid and Medicare guidelines.
  • Capability to work independently as well as collaboratively.
  • Willingness to engage in on-call duties.
