Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Family Health Centers of San Diego. As a key member of our healthcare team, you will provide high-quality, patient-centered care to our diverse patient population.

Responsibilities
  • Provide comprehensive medical services to patients, including health promotion, disease prevention, and management of chronic conditions.
  • Conduct thorough medical histories, physical examinations, and diagnostic tests to inform treatment plans.
  • Develop and implement individualized treatment plans, taking into account patient preferences, values, and cultural backgrounds.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Master's degree in Nursing with a focus on Family Nurse Practitioner or related field.
  • Current certification as a Family Nurse Practitioner (FNP) or related certification.
  • Current licensure as a Registered Nurse (RN) in the state of California.
  • Ability to work effectively in a fast-paced, dynamic environment with diverse patient populations.
  • Strong communication and interpersonal skills, with ability to work collaboratively with patients, families, and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
Preferred Qualifications
  • Experience working with underserved, low-income, or homeless populations.
  • Bilingual or multilingual skills, with ability to communicate effectively with patients from diverse linguistic and cultural backgrounds.
  • Knowledge of medical terminology, anatomy, physiology, and disease concepts.
  • Ability to work effectively in a team-based environment, with ability to prioritize tasks and manage time efficiently.
