We are seeking a dedicated Case Manager to join our team at St. Patrick Center, a ministry of Catholic Charities. This role is essential in providing comprehensive support to clients enrolled in our Permanent Supportive Housing Program.
Key Responsibilities:
- Deliver housing-centered case management and advocacy for individuals experiencing chronic homelessness.
- Develop and implement individualized treatment plans in collaboration with program managers.
- Conduct home visits to assess client needs and provide necessary support.
- Ensure stability and continuity of care for program participants.
Work Schedule:
The standard working hours for this position are Monday through Friday, from 8:00 AM to 4:30 PM.
Compensation:
The salary range for this role is between $40,000 and $50,000.
