Key Responsibilities- Personal Care and Hygiene: Assist patients with personal care needs, including bathing, dressing, and grooming, as assigned.
- Meal Preparation and Assistance: Plan and prepare nutritious meals, including shopping, as assigned, and assist patients with feeding as needed.
- Observation and Reporting: Observe patients and report any changes or concerns to the Case Manager, ensuring timely and effective care.
- Assistance with Daily Activities: Assist patients with daily activities, such as ambulation, exercise, and transfers, as instructed by the hospice nurse or therapist.
- Documentation and Communication: Accurately document patient care and communicate effectively with the healthcare team to ensure seamless care coordination.
- Respite and Support: Provide respite and support to patients, families, and caregivers, promoting a calm and comfortable environment.
- Household Maintenance: Maintain a clean and orderly living environment for patients, adhering to organization standards.
- Education and Training: Meet the training requirements of the State, including a minimum of 75 hours of classroom and supervised practical training.
- Age and Certification: Be at least 18 years old and possess current CPR certification.
- Communication and Interpersonal Skills: Demonstrate excellent oral and written communication skills, empathy, and tact, with the ability to work with diverse populations.
- Reliability and Flexibility: Be self-directing, with the ability to work with minimal supervision, and possess a reliable vehicle for transportation.
The Hospice Care Assistant will work in a community home environment, with occasional in-and-out-of-vehicle travel. The ideal candidate will be comfortable working in a fast-paced, dynamic environment and possess the ability to adapt to changing situations.
