Job Summary

We are seeking a skilled and compassionate Patient Care Manager/Field Registered Nurse Hybrid to join our team at Elite Home Health, a part of LHC Group. This hybrid role combines office and field work, providing a unique opportunity for professional growth and development.

As a Patient Care Manager, you will be responsible for supervising and coordinating clinical services, providing quality support to patient-facing teams, and ensuring the delivery of exceptional patient care. Your expertise and leadership skills will be essential in driving our mission to provide high-quality, cost-effective care to our patients.

Responsibilities

  • Supervise and coordinate clinical services to ensure timely and effective patient care.
  • Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Oversee and assure development, implementation, and updates to the individualized patient plan of care, as appropriate.

Requirements

  • Current RN licensure in state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
