Transition Liaison Specialist

Job Summary

We are seeking a highly motivated and experienced individual to fill the role of Transition Liaison Specialist. As a key member of our team, you will be responsible for providing quality communication and support to students transitioning from high school to employment. Your primary objective will be to facilitate a smooth transition by working closely with assigned schools, monitoring counseling and vocational services, and evaluating student progress towards employment goals.

In this role, you will ensure that vocational rehabilitation services are provided to individuals with disabilities to achieve competitive and integrated employment outcomes. You will monitor vocational counseling and guidance to determine eligibility for services and develop a vocationally focused plan for employment for eligible individuals. This will involve coordinating pre-employment transition services across the district, attending Individual Education Plan (IEP) meetings as necessary, and developing and maintaining relationships with schools and school systems in the district.
