Job Summary

Molina Healthcare is seeking a dedicated and compassionate IRIS Consultant to join our team. As an IRIS Consultant, you will play a vital role in supporting individuals with long-term care needs in achieving their goals and living independently.

Key Responsibilities
  • Provide program orientation to new participants, ensuring they understand their rights and responsibilities within the IRIS program.
  • Build and maintain professional relationships with participants, their families, and community resources.
  • Assist participants in identifying personal outcomes and developing customized IRIS plans to achieve their goals.
  • Research and connect participants with community resources and natural supports to enhance their quality of life.
  • Document all orientation and planning activities within the IRIS data system, ensuring timely and accurate reporting.
  • Collaborate with other IRIS Consultant Agency staff to ensure successful implementation of participants' plans.
Requirements
  • Bachelor's degree in a social work, psychology, human services, counseling, nursing, special education, or a closely related field.
  • 1+ year of direct experience related to the delivery of social services to individuals with intellectual or physical disabilities and older adults.
  • Excellent problem-solving skills, critical thinking skills, and strong basic math skills.
  • Ability to work independently, with minimal supervision, and be self-motivated.
  • Valid driver's license with a good driving record and reliable transportation.
