IRIS Consultant

Molina Healthcare is seeking a highly skilled and compassionate IRIS Consultant to join our team. As an IRIS Consultant, you will play a vital role in supporting individuals with long-term care needs to achieve their goals and live independently.

• Provide personalized support to IRIS participants, helping them navigate the Wisconsin IRIS program and access community resources.
• Conduct regular home visits and community outreach to build relationships and identify participant needs.
• Develop and implement customized IRIS plans, ensuring participants' goals are met while staying within their budget.
• Collaborate with participants, family members, and healthcare providers to ensure comprehensive care and support.
• Document all activities, progress, and changes in the IRIS data system, maintaining accurate and up-to-date records.
• Research and connect participants with community resources, natural supports, and services to enhance their quality of life.
• Communicate effectively with participants, families, and healthcare providers, ensuring clear understanding of program requirements and participant needs.
• Maintain confidentiality and adhere to HIPAA guidelines, ensuring the protection of sensitive participant information.

• Bachelor's degree in a social work, psychology, human services, counseling, nursing, special education, or a closely related field.
• 1+ year of direct experience in delivering social services to individuals with intellectual or physical disabilities and older adults.
• Ability to work independently, with minimal supervision, and be self-motivated.
• Knowledge of Long-Term Care programs and familiarity with principles of self-determination.
• Excellent problem-solving skills, critical thinking skills, and strong basic math skills.
• Excellent time management and prioritization skills to focus on multiple projects simultaneously and adapt to change.
• Ability to develop and maintain professional relationships and work through situations without taking it personally.
• Comfortable working within a variety of settings and adjusting style as needed, to work with a diverse population, various personalities, and personal situations.
• Resourceful and have knowledge of community resources while being proactive and detail-oriented.
• Ability to use a variety of technology, including but not limited to, Outlook, Skype, Teams, PowerPoint, Excel, Word, online portals, and databases.
• Valid driver's license with a good driving record and reliable transportation.
