Position Overview:

You will significantly impact the lives of individuals by providing essential nursing services, training, and support to meet their medical or psychiatric needs, particularly for those with brain injuries, intellectual disabilities, substance use disorders, or emotional challenges.
- Monitor the health status of individuals and ensure the delivery of high-quality services.
- Administer medications and maintain accurate documentation while supervising staff in medication administration.
- Implement prescribed medical treatments as per physician directives and provide personal care services.
- Conduct health assessments and develop healthcare services tailored to each Individual Service Plan (ISP), responding proactively to any changes in condition.
- Train staff on essential topics such as universal precautions, infection control, and medication administration.
- Act as a liaison and advocate among various medical and professional service providers.
- Educate individuals on self-medication, prescribed treatments, and health-related topics including sex education and smoking cessation.
- Associate's Degree in Nursing or a related field, or completion of an accredited LPN program.
- Minimum of one year of nursing experience with the population served.
- Current state LPN or LVN License.
- Valid driver's license, car registration, and auto insurance.
- Additional certifications as mandated by state regulations, including CPR/FA, HIV/AIDS certification, BBP, and ACT training.
- Strong communication skills to effectively interact with staff, individuals served, and their families.
