This role is responsible for maintaining and ensuring complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. The successful candidate will prepare, dispense, and verify medications ordered for an assigned group of patients, utilizing safe, rational, and cost-effective therapeutic decision-making. Key responsibilities include assessing, evaluating, and monitoring patient drug therapy, providing drug information, patient education, and other clinical pharmacy services.
Key Responsibilities- Evaluate all physician orders based on a clinical assessment of the patient, considering factors such as allergies, age, renal function, concomitant drug therapy, lab values, approved dosing guidelines, best practice protocols, diagnosis, and physical clinical status.
- Ensure the safe and accurate preparation and dispensing of medications, including sterile products and investigational drugs, as measured by QA, medication occurrences, competency assessment, and other documentation.
- Manage drug therapy to optimize patient outcomes, measured by intervention documentation, ADR assessment, chart reviews, target drug reviews, patient education, and the provision of drug information.
- Maintain professional and technical knowledge, licensures, and certifications through continuing education.
- Manage the operations of the pharmacy in compliance with state and federal regulations.
- Adapt behavior to the specific patient population, including respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions, and communication style.
- Bachelor's degree in Pharmacy or related field.
- Current Pharmacy license in the state of practice.
- Excellent interpersonal skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
- Computer skills and dexterity required for data entry and retrieval of patient information.
- Ability to work a flexible work schedule, including 24/7, weekend, holiday, and on-call availability.
The incumbent works in a healthcare setting with potential occupational risk for exposure to hazardous medications or hazardous waste. The risk level of exposure may increase depending on the essential job duties of the role.
