About the Role

Fountain House is seeking a highly skilled and compassionate Mental Health Social Practitioner to join our team. As a key member of our community, you will play a vital role in supporting individuals with serious mental illness to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Member Support and Advocacy
    • Engage with members in a community-based approach to therapy, focusing on skills development, confidence building, and social connections.
    • Develop and implement therapeutic interventions to address common barriers such as social isolation, poor self-esteem, anxiety, and symptom management.
    • Provide access to environments outside of Fountain House to build general skills and support members' self-efficacy.
    • Establish relationships with members and the community to leverage support for positive change and risk-taking.
    • Engineer various environments and social interactions to create therapeutic results and develop a positive community.
  • Training Support and Entitlements
    • Provide vocational training and support for transitional employment opportunities.
    • Assist members with entitlements and benefits, including food stamps, SSI/SSDI, Medicaid/Medicare, and housing applications.
    • Advocate for members' well-being, safety, and livelihood, addressing clinical, legal, housing, and independent living needs.
    • Complete documentation, including progress notes and reach-out documentation, in a timely manner.
Requirements
  • 1-3+ years' experience working with individuals with serious mental illness, preferably in a community setting.
  • 1-2 years' coaching and counseling experience, with a focus on mental health crisis management.
  • 1-2 years of vocational rehabilitation, training, or employment specialist experience.
  • Ability to effectively manage and handle member concerns and behaviors.
  • Proven experience with mental health crisis management and supporting individuals under emotional distress.
Preferred Qualifications
  • Undergraduate degree or an equivalent combination of training and experience.
  • Graduate degree or equivalent preferred.
  • Driver's License and the ability to drive a van preferred.

Salary and Benefits
