Job Summary:
We are seeking a highly skilled Psychiatric Mental Health Nurse Practitioner to join our team at the POSTGRADUATE CENTER FOR MENTAL HEAL in New York, NY. The successful candidate will be responsible for conducting initial psychiatric evaluations, participating in treatment planning, and managing ongoing medication needs for established clients.
Key Responsibilities:
Conduct initial psychiatric assessments, diagnosis, and updates as clinically necessary
Provide ongoing medication management through E-prescribing
Educate individuals and families about medications and their effects
Coordinate consumer access to medications with the RN and LPN, including administering injectable medications when needed
Order routine blood work and/or toxicology testing as needed
Document all services and outreach efforts via progress notes in the EHR within 24 hours
Participate in the development of person-centered treatment plans and sign off on coordinated care
Provide medical consultation to a multi-disciplinary CCBHC staff
Requirements:
NYS License in Psychiatric Mental Health Nurse Practitioner
Experience working with children and adults with mental health and substance use disorders
Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred
Excellent communication and leadership skills
