As a Licensed Practical Nurse (LPN), you will play a vital role in delivering safe and effective patient care under the guidance of a Registered Nurse (RN). Your responsibilities will encompass assisting in the nursing process, which includes monitoring, observing, and reporting on patient responses to their care plans. You will perform various tasks and skills that align with the scope of practice for a licensed practical nurse.

Key Responsibilities:

  • Work collaboratively with the healthcare team to ensure optimal patient care, consulting with the supervisory RN and other team members as necessary.
  • Seek guidance and supervision when implementing new or unfamiliar nursing practices.
  • Contribute to the admission assessment process by accurately collecting and documenting both objective and subjective data.
  • Support a multidisciplinary approach to identify appropriate patient care interventions that promote comfort and safety.
  • Assist the healthcare team in achieving patient care outcomes and goals.
  • Administer medications and perform tasks within your scope of practice.
  • Engage with patients and families to facilitate safe transitions in care.
  • Promptly inform the supervisory RN of any changes in patient conditions.
  • Collaborate with the RN and healthcare team to communicate progress towards established care goals.
  • Ensure real-time documentation of patient care in accordance with hospital policies.
  • Conduct thorough hand-offs with a focus on safety and detail.
  • Communicate effectively with providers during routine rounds and as needed.
  • Integrate patient and family education throughout the hospitalization, covering the care plan, procedures, and discharge instructions.
  • Incorporate preventative health measures into the educational framework.
  • Assess and select appropriate teaching strategies based on the patient's age, culture, and readiness to learn.
  • Consult with hospital resources for specialized education as needed.

Qualifications:

  • Completion of an accredited LPN program.
  • Prior experience as a CNA or PCT is preferred.
  • Current LPN licensure in the State of Iowa or a compact state.
  • Fluency in English, including reading, writing, and comprehension.
  • Proficiency in manual skills and techniques necessary for basic nursing procedures and specialized equipment operation.
  • Ability to complete training on the Electronic Medical Record (EMR) system.
  • Understanding of the laws governing licensure and practice within legal boundaries.
  • Adherence to established department routines, policies, and procedures.
  • Strong verbal and written communication skills with patients, families, and healthcare team members.
  • Real-time EMR documentation in line with hospital and unit policies.
