Job Summary

We're seeking a Customer Success Liaison to work on the front lines with physicians and clinical teams, delivering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services. As a key contributor to our mission, you'll ensure patients have access to our innovative care solutions.

Key Responsibilities
  • Build relationships with clinicians and their office staff to support patient enrollments in Nsight's RPM and CCM services.
  • Drive success by facilitating high-value, patient-centered care, working directly in clinics to enroll patients, organize data, and enhance communication between providers and patients.
  • Educate and engage patients and clinical teams, providing education about Nsight's care programs and driving engagement.
  • Champion change by implementing team-based, patient-centered processes that improve chronic disease management.
  • Optimize outcomes by collaborating with care teams to ensure timely onboarding and care plan adherence.
Requirements
  • 5-10 years of experience in customer success, account management, or client services, preferably in healthcare.
  • A track record of driving patient enrollments and success in a fast-paced, client-facing role.
  • Strong communication and relationship-building skills, with the ability to educate and inspire.
  • Experience with HubSpot, EHR/EMR systems, and customer service tools a plus.
  • Bilingual in English and Spanish is a plus.
