Job Title: Licensed Practical Nurse

At Northern Rivers Family of Services, we are seeking a highly motivated and experienced Licensed Practical Nurse (LPN) to join our team in our residential program in Schenectady, NY. As an LPN, you will play a vital role in providing medical care and services to a youth and adolescent population with emotional, behavioral, and physical needs.

Responsibilities:
  • Coordinate medical care, dental care, and mental health services for our clients
  • Ensure that medical treatment services meet the requirements of the regulating agency
  • Review lab and test results, reporting critical values to RN and providers
  • Manage medications, including ordering and obtaining prior authorizations
  • Dispense medications and provide client and family education regarding medications and treatments
  • Collaborate with our staff regarding concerns and behaviors
  • Participate in Interdisciplinary Medication Review meetings
  • Nursing documentation using EHR
  • Update discharge planning needs
Requirements and Education:
  • LPN Licensed in New York State
  • 1 to 2 years of experience
  • Experience working with youth and adolescent population desired
  • Must pass physical
  • Must be able to complete TCI training
