Our Caregivers are essential members of our team, providing vital support to individuals in their homes while actively engaging in care planning alongside our interdisciplinary team (IDT).

Position Overview:

  • The Caregiver role, under the supervision of the Home Care Supervisor, focuses on delivering essential personal care services to participants in their residences, adhering to a structured schedule and established guidelines.

Primary Duties:

  • Deliver direct personal care and assist with daily living activities (ADLs) such as bathing, dressing, grooming, toileting, and lifting as necessary.
  • Support Instrumental Activities of Daily Living (IADLs) including laundry, light housekeeping, and grocery shopping.
  • Monitor and document participants' vital signs, assist with meal preparation and feeding, ensure adequate hydration, facilitate safe transfers, and accompany participants to medical appointments.
  • Collaborate with the Rehabilitation department on home exercise programs and provide translation services when needed.
