As integral members of our interdisciplinary team (IDT), we are committed to providing exceptional care while fostering a positive work atmosphere.

Under the supervision of the Home Care Supervisor, the Home Care Assistant is responsible for ensuring the safe provision of personal care services to participants in their homes, as per the established schedule and directives.

Key Responsibilities:

  • Deliver hands-on personal care and assist with daily living activities (ADLs) for participants, including bathing, grooming, dressing, toileting, and safe transfers.
  • Support participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Monitor and record participants' vital signs as required.
  • Assist with light meal preparation and feeding, ensuring participants receive adequate nutrition and hydration.
  • Facilitate safe transfers in and out of the home, and provide medical escort services as needed.
  • Collaborate with the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.
  • Offer translation services when qualified, as necessary.
