We are seeking a compassionate and experienced Patient Care Manager - Hospice to join our team at Northern Arizona Hospice in Flagstaff, AZ. As a key member of our clinical team, you will play a vital role in ensuring the delivery of safe and efficient patient care that adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.
Key Responsibilities:- Liaise between staff, patients, and the medical community to ensure seamless care coordination.
- Lead, inspire, and motivate others to provide exceptional care.
- Ensure all staff are oriented and supported to be successful in their job performance.
- Actively promote and direct assigned team regarding quality of care and safety of patients and staff.
- Engage in thorough problem resolution and complaint investigation.
- Provide in-services to staff to promote the health and safety of patients and familiarize staff with regulatory issues and agency policy and procedures.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
