About the Position

We are seeking a highly skilled Medical Care Specialist to join our team at Mass General Brigham in Randolph, Massachusetts.

Job Summary

The ideal candidate will provide quality clinical services to patients in various settings within the community. They will deliver patient care within our organizational philosophy, policy, and standards of community health practice. The admissions process and case management responsibilities will be performed in accordance with MGB Healthcare at Home policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Skilled nursing care will be provided in the form of assessment, teaching, treatment, documentation, and care coordination, demonstrating high-quality customer service and financial awareness.

Key Responsibilities
  • Assess and plan individualized patient care plans
  • Deliver skilled nursing care in a variety of settings
  • Collaborate with healthcare professionals to coordinate care
  • Communicate effectively with patients, families, and healthcare teams
Requirements and Qualifications

To be considered for this role, applicants must meet the following requirements:

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in an acute care setting
  • Prior home care experience is preferred

Compensation

The estimated salary for this position is $80,000 - $110,000 per year, depending on experience.
