Job Summary

The Hospice Clinical Liaison is a key member of the Compassus team, responsible for promoting the company's values and philosophy while providing exceptional care to patients and their families. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with healthcare professionals and community organizations.

Key Responsibilities
  • Community Outreach and Education: Develop and maintain relationships with healthcare providers, community organizations, and other stakeholders to promote Compassus services and educate them on the benefits of hospice care.
  • Patient Assessment and Eligibility: Conduct thorough assessments of patients' clinical needs and disease status to determine eligibility for Compassus services.
  • Admission and Discharge Coordination: Facilitate the admission process for patients, including obtaining necessary documentation and coordinating with healthcare providers and community organizations.
  • Collaboration with Healthcare Providers: Work closely with attending physicians, hospice physicians, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Marketing and Promotion: Develop and implement marketing strategies to promote Compassus services and increase market share.
  • Quality Improvement and Compliance: Participate in quality improvement initiatives and ensure compliance with regulatory requirements and company policies.
Requirements
  • Education and Experience: RN license required; Bachelor's degree in Nursing highly preferred; two to three years of related experience required.
  • Skills and Abilities: Strong communication and interpersonal skills; ability to work effectively in a fast-paced environment; proficiency in Microsoft Office and electronic medical records systems.
  • Certifications and Licenses: Active and unencumbered Registered Nurse license in state(s) of employment required.
