Key Responsibilities- Assess and Coordinate Care: Conduct thorough assessments of patients' physical, psychosocial, and environmental needs, and develop individualized plans of care to ensure quality and continuity of care.
- Provide Skilled Nursing Services: Furnish skilled nursing services, including wound care, medication management, and education to patients and their families.
- Coordinate Services: Work closely with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless coordination of care and services.
- Communicate Effectively: Provide clear and concise communication to patients, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes.
- Supervise and Educate: Supervise and educate home health aides, LPNs/LVNs, and other nursing personnel to ensure high-quality care and adherence to agency policies and procedures.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current State license as a Registered Nurse and current Driver's License.
- Experience: One year of experience as a Registered Nurse in a clinical care setting, with home health experience preferred.
- Knowledge and Skills: Nursing skills as defined by generally accepted standards of practice, good interpersonal skills, and proof of current CPR.
- Transportation: Reliable transportation and valid and current driver's license and auto insurance.
