Job Description
This role involves working with an interdisciplinary team to develop plans of care that meet the needs of our participants. The ideal candidate will have experience working with frail or elderly populations and possess strong communication skills.
Responsibilities
- Develop and implement nutritional care plans for program enrollees
- Provide direct participant care and education as needed
- Collaborate with the team to develop plans of care
- Establish follow-up systems and maintain accurate documentation
Required Qualifications
- Registered Dietitian with licensure in Ohio required
- Bachelor's degree in nutrition and dietetics, or master's degree preferred
- Minimum one year's experience working with frail or elderly populations required
- Experience in community-based programs and interdisciplinary care settings desirable
- Serv Safe Certification desired
- Current Ohio driver's license and reliable transportation required
Benefits Package: Comprehensive benefits package including paid time off, sick time, holidays, health insurance, life insurance, and retirement plan.
Salary Estimate: $70,000 - $85,000 per year based on experience and qualifications.
