We are in search of a Licensed Physical Therapy Specialist to become part of our West Union, SC team. We provide specialized services, including Manual Therapy, Fall Prevention, Vestibular Rehabilitation, LSVT/BIG, Orthopedic, Worker's Compensation, and Cancer-Related Fatigue. LifeStrides Physical Therapy & Rehabilitation is well-regarded for its strong patient population and its commitment to delivering effective, personalized healthcare solutions.Key Responsibilities:
1. Evaluate patients and formulate and initiate treatment plans based on assessment results.
2. Review the Physician's referral and the patient's medical history to determine necessary physical therapy interventions.
3. Deliver direct patient care in accordance with established protocols and guidelines.
4. Document findings, progress, and instructions for patients and caregivers.
5. Oversee patient care provided by assistants and rehabilitation technicians.
6. Develop a written plan of care, establish goals based on evaluation findings, and create a comprehensive program to achieve those goals.
8. Communicate effectively with patients, caregivers, and professional colleagues, including physicians and insurance representatives.
9. Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.
Qualifications:
1. Graduate from a CAPTE-accredited Physical Therapy program.
2. Current South Carolina state license and CPR certification.
3. Ability to create and implement outpatient balance and vestibular rehabilitation treatment plans.
4. Capable of working efficiently and effectively in an autonomous environment.
5. Proficient in basic PC skills and knowledge, including Electronic Medical Record (EMR) systems, database software, and MS Office.
