About the Job

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will be responsible for assessing, planning, implementing, and evaluating patient care based on the standards of care and policies of LCMC Health.

You will facilitate and maintain a safe environment for patients and others with a specialty in wound care treatment and prevention. This includes performing priority technical procedures safely, effectively, efficiently, and legally, as well as actively promoting and supporting LCMC Health's performance improvement initiatives.

As a Nurse Care Specialist, you will utilize the nursing process to deliver patient care. This includes organizing, prioritizing, and coordinating all aspects of care for each assigned patient by coordinating and delegating activities related to patient care management. You will also communicate using appropriate channels information, data, significant events, or problems to appropriate persons, including current patient condition, changes in the patient's condition, laboratory/diagnostic data, intake & output, nutrition needs, assessment findings, and changes.

About the Role
  • Provide direct patient care and coordinate with interdisciplinary team members to identify actions to improve patient care and education.
  • Seek supervision, consultation, and assistance when unable to perform effectively or safely independently.
  • Delegate safely and appropriately to other team members to accomplish direct patient care or support activities.
  • Consistently perform proper hand-off to ensure continuity of care while identifying and communicating problems.
  • Advise supervisor of current or anticipated problems and accepts and completes assignments in a timely fashion.
