Nursing Care Specialist

Job Title: Registered Nurse (Home Health) - Taos, NM_Full Time Perm
Shifts: Monday - Friday, 8:00 AM to 5:00 PM, with flexibility based on patient needs.
- Be available for on-call duties on weekends, with on-call pay of $75 per day
- Patient ratio of 4-6 per day
We are hiring experienced Registered Nurses for Home Health and Hospice roles. Candidates should have 1-2 years of relevant experience, proficient in Kinsser/WellSky.
Responsibilities:
- Conduct comprehensive assessments of patients in their homes, ensuring individualized care plans are developed and implemented.
- Administer skilled nursing care to patients requiring various medical treatments and interventions.
- Collaborate with interdisciplinary teams to coordinate and manage patient care effectively.
- Utilize Kinsser/WellSky for documentation and communication to ensure accurate and timely record-keeping.
- Provide support to patients ranging from middle age to geriatric, adapting care plans to meet their unique needs.
- Maintain a personalized approach to deliver quality care.
- Respond promptly to changes in patient conditions and modify care plans accordingly.
- Collaborate with other healthcare professionals to ensure a holistic approach to patient care
Qualifications: - RN license and CPR certification
- 1-2 years of Home Health and Hospice experience
- Proficiency in Kinsser/WellSky preferred
- Strong communication and interpersonal skills
