- Client Advocacy and Case Management:
- Advocate for clients to ensure they have access to necessary resources, services, and support systems.
- Coordinate care with healthcare providers, social services, and community resources to ensure comprehensive support for clients.
- Engage in discharge planning and follow-up to ensure continuity of care and successful transitions.
- Documentation and Compliance:
- Maintain accurate and timely clinical records in accordance with state regulations, ethical guidelines, and organizational policies.
- Ensure compliance with all licensure requirements, confidentiality standards, and documentation protocols.
- Participate in regular case reviews, supervision, and continuing education to maintain licensure and stay current with best practices.
- Collaboration and Communication:
- Collaborate with a multidisciplinary team, including psychiatrists, nurses, and other social workers, to provide integrated care to clients.
- Participate in team meetings, case consultations, and professional development activities.
- Communicate effectively with clients, families, and colleagues to foster a positive and supportive therapeutic environment.
- Education:
- Master's degree in Social Work from an accredited institution.
- Licensure:
- Active licensure in Minnesota as a Licensed Independent Clinical Social Worker (LICSW).
- Experience:
- A minimum of 3 years of post-graduate clinical experience.
- Additional Requirements:
- Ability to work a hybrid schedule, with some on-site presence required.
- Commitment to maintaining confidentiality and adhering to professional ethical standards.
- Flexibility to occasionally adjust work hours to meet client needs.
- Competitive salary within the range of $74,000 - $98,000 annually, based on experience and qualifications.
