Home Care RN Opportunity

We are seeking an experienced Registered Nurse to join our team as a Home Care RN. As a Home Care RN, you will provide skilled nursing care to patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
  • Administer medications and treatments as prescribed by physicians, monitoring patients' responses and adjusting care plans as needed.
  • Educate patients and caregivers on disease management, medication use, and wound care, promoting independence and self-care.
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes, utilizing our electronic medical record system.
Requirements:
  • Current RN licensure in the state of Connecticut.
  • At least 1 year of clinical experience, with a strong background in home care and Oasis experience preferred.
  • Ability to work independently, with strong communication and interpersonal skills.
