Speech Language Pathologist

Constellation Health Services is seeking a skilled Speech Language Pathologist to join our home care team. As a key member of our clinical team, you will be responsible for delivering high-quality speech therapy services to patients in their homes.
Key Responsibilities:
- Conduct comprehensive evaluations and assessments to identify speech and language deficits.
- Develop and implement individualized treatment plans to improve patient outcomes.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Maintain accurate and timely documentation of patient progress.
Requirements:
- Master's degree in Speech-Language Pathology from an accredited program.
- Current licensure and registration as a Speech-Language Pathologist in the state of Connecticut.
- At least 1 year of clinical experience in home care or a related setting.
