Private Duty Nurse

The Private Duty RN is a skilled nursing professional responsible for providing high-quality patient care in the comfort of their own homes. As a key member of the Health First team, you will be accountable for delivering exceptional customer experiences and ensuring the well-being of our patients.
Key Responsibilities- Develop and implement individualized plans of care for patients, in collaboration with physicians and other healthcare professionals.
- Provide skilled nursing care, including wound care, medication management, and education on disease management and prevention.
- Conduct thorough assessments of patients' physical, emotional, and cognitive needs, and develop plans to address these needs.
- Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
- Supervise and train Home Health Aides to ensure high-quality patient care.
- Document patient progress and outcomes in a timely and accurate manner, in accordance with regulatory requirements.
- Current RN licensure in the state of practice.
- Minimum 1 year of experience in home health care or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong focus on customer satisfaction and quality care.
