Palliative Care Nurse Specialist

Job SummaryWe're seeking an experienced Palliative Care Registered Nurse to join our team. The ideal candidate will have a deep commitment to serving patients and families with love and compassion, ensuring every individual receives high-quality care and support.
Key Responsibilities- Evaluate patients and create personalized care plans to address their unique needs
- Communicate effectively with patients, families, and the care team to ensure seamless coordination of care
- Promote patient comfort and well-being through empathetic and compassionate care
- Graduate of an accredited nursing school with a current state RN license
- Minimum of 2 years nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation and ability to lift at least 25 lbs effectively
