Nurse Care Specialist

We are seeking a compassionate Registered Nurse to join our team as an After-hours Hospice RN on Weeknights. This salaried full-time position involves covering on-call hours from 5 pm - 8 am, Monday through Thursday, and ending at 8 am on Fridays.
Job OverviewThis role is perfect for nurses who value patient-centered care and want to make a meaningful difference in the lives of those facing life-limiting illnesses. As a member of our hospice care team, you will be part of a dedicated group of professionals who provide comfort, support, and exceptional care to patients and their families.
Key Responsibilities:- Providing high-quality nursing care to patients in their homes or other settings
- Serving as a patient advocate, ensuring that each individual receives the quality care they deserve
- Collaborating with the hospice care team to develop and implement personalized care plans
- Maintaining accurate and detailed records of patient interactions and care provided
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- At least 2 years of nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation
