- Collaborate with healthcare professionals to ensure continuity of care and achieve optimal patient outcomes.
- Provide skilled nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Document patient care and progress accurately and timely, in collaboration with physician orders.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
