At Phoenix Home Care and Hospice, we are committed to providing exceptional care to our clients and their families. As a Hospice Registered Nurse Case Manager, you will play a vital role in delivering patient-centered care and supporting our interdisciplinary team.
Key Responsibilities:
- Provide education and guidance to patients and families on the hospice care process and plan of care.
- Collaborate with the attending physician and Hospice Medical Director to ensure seamless care transitions.
- Coordinate hospice care services, working closely with other team members and patients/families to meet their needs.
- Conduct comprehensive assessments and evaluations to ensure high-quality care.
Requirements:
- Kansas RN License
- Hospice experience preferred but not required
- Valid driver's license and auto liability insurance
