As a Case Manager II, you will play a vital role in serving as a primary support and case manager for individuals with mental illness and other co-occurring disorders.

**Job Summary**

Under the direct supervision of our experienced team, you will assist individuals to live as independently as possible in their chosen community. This role requires a high level of emotional intelligence, empathy, and strong communication skills to build trusting relationships with our clients.

**Key Responsibilities**

• Provide primary support and case management services to individuals with mental illness and other co-occurring disorders

• Assist clients in achieving their goals and living independently in their chosen community

• Develop and implement individualized service plans that address client needs and goals

• Collaborate with a multidisciplinary team to provide comprehensive care and support

**Requirements**

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services

• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver

• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
