About the Role
At Virginia Mason Medical Center, we are seeking a skilled RN Care Manager to join our Hematology team. As a key member of our care team, you will play a vital role in coordinating and facilitating quality management of our outpatient population across the continuum of care.
Responsibilities
- Coordinate and facilitate quality management of a broad-based outpatient population across the continuum of care.
- Provide skilled triage and assessment, education, collaboration, and coordination of healthcare and community resources.
- Support the care team in the provision of patient care.
- Provide patient advice and guidance.
- Assist in prioritizing daily work of the clinic.
- Provide direction to team members, including oversight of Medical Assistant's clinical competency.
Qualifications
- Current Washington State licensure as an RN.
- CPR certification is required every two years.
- Graduation from an accredited school of nursing.
- Strong patient assessment skills and ability to work collaboratively in a team setting.
- Current working knowledge of electronic medical records.
