Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a key member of our team, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes.

Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, diagnosis, and treatment planning
  • Provide health education and counseling to patients and their families
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure comprehensive care
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
