We are seeking a skilled Physical Therapist to join our team at Aurora West Allis Medical Center - Outpatient Rehabilitation. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation program.
Key Responsibilities- Interpret physician referrals and conduct comprehensive evaluations to identify patient needs and develop individualized treatment plans.
- Implement evidence-based interventions to promote patient recovery and functional independence.
- Monitor patient progress, adjust treatment plans as needed, and communicate effectively with healthcare teams to ensure seamless care coordination.
- Collaborate with patients, families, and caregivers to educate them on self-management techniques, home programs, and community resources.
- Maintain accurate and timely documentation, including patient progress notes, team conference reports, and discharge summaries.
- Basic Life Support (BLS) certification issued by the American Heart Association (AHA) within 6 months of hire.
- Physical Therapist license issued by the state of Wisconsin.
- Bachelor's Degree in Physical Therapy.
- Intermediate computer skills and excellent communication and interpersonal skills.
This role requires the ability to sit, stand, walk, lift, and squat throughout the workday. The Physical Therapist must be able to lift up to 50 lbs. from floor to waist, lift up to 10 lbs. from waist to overhead, and lift and carry up to 40 lbs. at waist height a reasonable distance. The role also requires pushing/pulling with 30 lbs. of force, performing a sliding transfer of 150 lbs. with a second person present, and repetitive use of hands for simple grasping, pushing/pulling, and fine manipulation.
This position may require travel, exposing the Physical Therapist to road and weather hazards. The role also involves operating all necessary equipment to perform the job.
