Job Title: Physical Therapist - Float

Advocate Aurora Health is seeking a per diem / prn Physical Therapist for a per diem / prn job in West Allis, Wisconsin.

Major Responsibilities:
  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals. Monitor and evaluate outcomes and adjust the individualized treatment plan as needed.
  • Plan for and complete patient discharge, including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
  • Complete all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate.
  • Responsible for cleaning up after patient treatment, putting away equipment, and maintaining a clean and organized workspace.
  • Act as a clinical and department resource, mentoring and facilitating clinical problem-solving with other staff.
  • Participate in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and improve quality of care.
  • Pursue professional growth through participation in continuing education courses, department inservices, and reading professional literature.
Licensure, Registration, and/or Certification Required:
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months.
  • Physical Therapist license issued by the state of Wisconsin.
Education Required:
  • Bachelor's Degree in Physical Therapy.
Experience Required:
  • Typically requires 4 years of experience in physical therapy.
Knowledge, Skills & Abilities Required:
  • Clinical expertise in specific areas of rehabilitation or proficient in a broad spectrum of care.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.
Physical Requirements and Working Conditions:
  • Must be able to sit, stand, walk, lift, and squat throughout the workday.
  • Must be able to lift up to 50 lbs. from floor to waist, lift up to 10 lbs. from waist to overhead, and lift and carry up to 40 lbs. at waist height a reasonable distance.
  • Must be able to push/pull with 30 lbs. of force, perform a sliding transfer of 150 lbs. with a second person present, and position requires repetitive use of hands.
