Nurse Navigator Interventional Radiology Specialist

The UNC Health Care department of Vascular Interventional Radiology is seeking a skilled nursing professional to join the VIR team as a Nurse Navigator. This role will facilitate enhanced patient experience, communication, and coordination of care.
Key Responsibilities- Ensure continuity of communication between a patient's various care providers
- Coordinate patient care duties, including interviews, chart reviews, and physical assessments
- Develop and implement personalized care plans for patients
- Collaborate with interdisciplinary teams to provide comprehensive care
- Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing
- Licensed to practice as a Registered Nurse in the state of North Carolina
- Basic Life Support (BLS) for Healthcare Provider certification
- Two (2) years of nursing experience and one (1) year in the area of specialization
UNC Health Care offers a competitive salary and benefits package, including PTO accrual and a generous benefits package. This is a full-time position with 40 hours/week and 8-hour weekday shifts. The position qualifies for UNC's benefits package.
