About the Role

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as an On Call/After Hours RN. As a member of our Home Health and Hospice team, you will provide high-quality patient care and support to individuals in their homes, focusing on their physical, emotional, and spiritual well-being.

Key Responsibilities
  • Provide skilled nursing assessments, interventions, and supportive care to home health and hospice patients
  • Respond to clinical and non-clinical situations after normal business hours
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies
  • Document patient progress, outcomes, and any changes in condition
Requirements
  • Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN)
  • Three years experience in acute care, medical-surgical, oncology, or geriatric nursing
  • Experience in a Medicare-certified home health agency preferred
  • Valid driver's license, insurance, and acceptable driving record
