The Licensed Practical Nurse (LPN) specializing in Complex Care Management will play a vital role in supporting the care management team by addressing the healthcare management needs of patients served by Cornell Scott Hill Health Corporation.
This position involves Care Coordination and Management, where the LPN will collaborate with Medical Providers, Behavioral Health Providers, Complex Care Managers, and members of the Interdisciplinary Team to deliver clinical support services and manage patient data effectively.
Additionally, the LPN will assist clients in navigating entitlement programs, resource allocation, and housing challenges. Responsibilities also include managing referrals and follow-ups, as well as collecting, recording, and reviewing patient data. Administrative support through office and phone tasks will be provided to the care management team as necessary.
Key Responsibilities:
- Contributes to the creation of a comprehensive care management plan, focusing on patients with complex medical needs.
- Collaborates with patients, caregivers, and co-learners to establish care goals, notifying the care team if a patient is unsuitable for complex care and suggesting alternative care plans.
- Delivers clinical support and direct care management, which includes patient education, goal-setting, and coaching for high-risk individuals.
- Engages with patients and their caregivers to ensure seamless care continuity, adherence to care plans, and identification of any barriers to compliance.
- Works alongside the care management team to pinpoint client needs for advocacy in accessing health and behavioral health services, entitlements, and community resources.
- Documents and communicates client requirements to the care management team.
- Performs additional duties as required.
Qualifications:
- Graduation from an accredited LPN program with current licensure in Connecticut.
- A minimum of one (1) year of experience in a long-term care or rehabilitation setting is preferred; clinic experience is a plus.
- Strong patient management skills are essential.
- Self-motivated with the ability to work under supervision.
- Proficient computer skills are necessary, with the ability to learn various healthcare management systems.
- Experience in phone triage, knowledge of referral processes, medication refill procedures, and charting is required.
- Familiarity with CLIA waived lab testing, urine dipsticks, and glucometers is important.
- Bilingual candidates are encouraged to apply.
