Pharmacist - Long-Term Care Specialist

PharMerica is seeking a highly skilled and motivated pharmacist to join our team. As a pharmacist with PharMerica, you will have the opportunity to provide exceptional patient care and work in a dynamic and growing organization.
About the RoleThis position will provide the opportunity to work in a closed-door pharmacy setting, providing essential care to patients under the direct guidance of a state-licensed pharmacist. You will be responsible for evaluating physician medication orders, maintaining patient databases, and collaborating with the healthcare team to provide clinical and technical support.
Responsibilities- Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed.
- Maintain patient/resident databases to include current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure that appropriate laboratory tests are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail-oriented; goal-oriented, team player, oral communication.
