Job Summary

Pathways Hospice is seeking a skilled RN Clinical Liaison to join our team. As a liaison between hospitals, facilities, and physicians, you will build effective relationships to promote communication, problem-solving, and positioning the affiliate as a premier high-quality hospice and palliative care provider.

Key Responsibilities
  • Acts as a liaison with assigned hospitals, facilities, and physicians to promote effective communication and problem-solving.
  • Provides assessment, supportive care, and education to prospective patients and families regarding hospice services.
  • Determines hospice appropriateness of the patient and assists in admitting patients to the hospice program.
  • Assesses and counsels patients and families during initial interviews to determine the level of care and physical and psychosocial needs.
  • Efficiently and accurately completes all admission paperwork and initiates the clinical record through the EMR process.
  • Assesses and places orders for immediate medication needs and oversees the placement of orders for durable medical equipment, supplies, and oxygen as needed.
  • Communicates pertinent patient/family information to the interdisciplinary team and facilitates a smooth transition.
Requirements
  • Graduate of an accredited school of nursing; BSN preferred.
  • 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care, or home health.
  • Current Colorado RN licensure and valid Colorado State driver's license and auto insurance.
  • CPR certification.
